Our Story

J.A.S.O.N C.A.R.E. is a registered business name of JC Healthcare Group Pty Ltd and has been registered since 31 July 2022. The organisation was created with a clear purpose: to provide nurse-led care and coordination that combines clinical understanding with dignity, responsiveness and genuine personal attention.

We aim to help people feel informed, respected and supported while navigating healthcare, disability, injury and community service systems. We support clients across private and eligible funded care pathways, subject to individual circumstances, approvals, service suitability and availability.

MEET OUR FOUNDER

Meet Jason, Registered Nurse and Founder

Jason Chua is a Registered Nurse with more than 16 years of healthcare experience across emergency care, occupational health, injury management, community healthcare, case management and care coordination. His professional experience includes working with clients, families, healthcare providers, employers and insurers, as well as supporting people navigating TAC, WorkCover and disability-related care systems.

Jason founded J.A.S.O.N C.A.R.E. to create a more personal, clinically informed and coordinated model of care—one that respects each person’s circumstances, preferences and goals.

Initial Assessment

Personalised Support Plan

Service Commencement

We review the enquiry and contact the client or referrer to understand the initial needs.

A Registered Nurse or suitable team member completes an assessment based on the requested service and individual circumstances.

We recommend a tailored plan focused on safety, care goals, preferences and available funding.

Once suitability, consent, funding and availability are confirmed, services can begin.
